Application Details for Registration in the Register of Dentists:

1. For dentists qualified from Ireland :

Please return the completed registration form together with:

• A copy of your Degree certificate.
• A letter of good standing from the Registration Board in whose jurisdiction you last practised dentistry.
• Remittance of €150.00.
  

2. For dentists qualified from other EEA Member States:

Please return the completed registration form together with:

• A copy of your Degree certificate.
• A certificate from the Competent Authority in the Member State of origin confirming that your formal qualification as a dental practitioner is referred to in Annex V (5.3.2) of the Professional Qualifications Directive 2005/36/EU and which satisfies the minimum training conditions referred to in Article 34 of this Directive. This certificate, if in a language other than in English, should be translated into the English language and duly notarised. Both original and translation should be forwarded.
• A letter of good standing from the Registration Board in whose jurisdiction you last practised dentistry.
• Proof of nationality of an EEA State.
• Remittance of €150.00.

3.  For dentists entitled to benefit from Mutual Recognition agreement with Canada (graduates after 5 December 2012).

Please return the completed registration form together with:

• A copy of your Degree certificate.
• If you are already registered, you must provide a Letter of Good Standing from the Registration Board where you last practiced. 
• If you are a new graduate who has not registered with a Registration Board you must have the Dean of your programme sign the Character Reference on the application form. 
• A certified copy of your passport 
• Remittance of €150.00.

Any documentation, if in a language other than in English, should be translated into the English language and duly notarised. Both original and translation should be forwarded.
